Planning fertility treatment in Japan? This article shares the real experience of a couple in their 40s who underwent IVF and ICSI treatment at Japanese fertility clinics. Based on actual medical records, we explain the treatment process, costs, and our personal observations to help international patients better understand what fertility treatment in Japan is like.

Does Fertility Treatment Change After 40?

For women in their 20s and 30s, fertility treatment usually begins with:

  • Timed intercourse

  • Intrauterine Insemination (IUI)

However, once patients reach their 40s, many fertility specialists recommend moving directly to assisted reproductive technologies due to the natural decline in egg quality associated with age.

The most commonly recommended treatments include:

  • IVF (In Vitro Fertilization)

  • ICSI (Intracytoplasmic Sperm Injection)


Common Fertility Treatments in Japan

Intrauterine Insemination (IUI)

IUI involves placing prepared sperm directly into the uterus.

It is minimally invasive, generally painless, and often considered before IVF for younger patients.

In Vitro Fertilization (IVF)

During IVF, eggs are retrieved from the ovaries and fertilized with sperm in a laboratory. The resulting embryo is then transferred into the uterus.

Intracytoplasmic Sperm Injection (ICSI)

ICSI is an advanced form of IVF in which a single sperm is injected directly into an egg using a microscope.

This method is commonly recommended for:

Clinic A (ART & ICSI Specialist)

Clinic A specializes in assisted reproductive technology (ART), including IVF and ICSI.

Initial Consultation

February 20

Cost: Free

The first visit consisted of approximately one hour of counseling with administrative staff, explaining the overall treatment process.

Day 2 of Menstruation

March 5

Cost: ¥10,340

Procedures included:

  • Blood tests for both partners

  • Signing consent forms

This officially marked the beginning of treatment.

Detailed Testing

March 15

Cost: ¥17,490

Wife

  • Ultrasound examination

  • Chlamydia screening

Husband

  • Semen analysis

The examination confirmed:

  • Low sperm count

  • Low sperm motility

Beginning the Egg Retrieval Cycle

May 2

Cost: ¥20,970

Detailed instructions were provided regarding:

  • Daily hormone injections

  • Oral medications

Daily Hormone Injections

Starting May 10

Daily self-injections began.

The main side effects included:

  • Nausea

  • Fatigue

  • Lightheadedness

Follicle Monitoring

Ultrasound examination followed by additional medication.

Cost: ¥9,540

Additional hormone injections were prescribed.

A nasal spray medication was also added.

Intravenous sedation was requested for the egg retrieval procedure.

Egg Retrieval

May 24

Cost: ¥23,160

Egg retrieval was performed under intravenous sedation.

The procedure itself lasted approximately 30 minutes.

Including recovery time, the entire visit took about 90 minutes.

Embryo Transfer

July 23

Cost: ¥45,070

Embryo transfer was performed without anesthesia.

Pregnancy Test

August 1

Cost: ¥1,330

Unfortunately, the pregnancy test was negative.

Referral Letter

Cost: ¥4,400

The referral letter had to be collected in person and could not be mailed.


Experience at Clinic A

Two physicians shared responsibility for patient care.

While the medical treatment itself was professional, explanations sometimes felt brief.

For example, when the husband visited to collect consent documents, he was simply told to "read them later" without much additional explanation.

Compared with another clinic we later visited, the overall treatment timeline also felt somewhat longer.

During embryo transfer, the catheter had to be replaced several times. After returning home, there was bleeding comparable to the first day of menstruation.

Although these experiences caused some anxiety, they reflect only our individual experience and may differ for other patients.

Clinic B (IVF)

Clinic B provides individualized fertility treatment tailored to each patient's condition.

Initial Consultation

August 15

Cost: Free

A referral letter from the previous clinic was submitted.

The physician spent approximately two and a half hours carefully explaining the treatment plan and answering questions.

Diagnostic Testing

September 1

Cost: ¥5,450

Husband

  • Semen analysis

Wife

  • Blood tests

  • Cervical cancer screening

September 5

Clomid treatment began.

September 6

Cost: ¥13,430

Subcutaneous hormone injection.

The injection was more painful than expected, and significant drowsiness occurred afterward.

Egg Retrieval

September 14

Cost: ¥14,680

On the day of egg retrieval, we brought:

  • A semen sample

  • Basal body temperature chart

The pain during retrieval was slightly stronger than typical menstrual cramps.

Frozen Embryo Results

November 15

Cost: ¥39,950

The physician explained the number and quality of embryos that had been successfully frozen.

Embryo Transfer

May 26

Cost: ¥42,860

Pregnancy Test

June 7

Cost: ¥1,970

Unfortunately, the result was negative.

Referral Letter

The clinic was able to mail the referral letter.

Cost: ¥1,140

Afterward, we transferred to another fertility clinic offering ICSI treatment.

Unlike our previous clinic, the same physician treated us throughout the entire process.

Each appointment included a thorough explanation of treatment options, helping us feel informed and reassured.

The egg retrieval procedure was smooth and well organized, and we felt comfortable throughout the experience.

What impressed us most was the physician's willingness to listen carefully and make treatment decisions together with us.
