This quotation form allows you to check the availability of a medical interpreter.

Payment must be completed within 24 hours of confirming the interpreter’s availability and at least one day before the service date.

This form is intended only for customers who are seriously considering booking our medical interpreting service.

Please note that uploading a copy of your passport is required when submitting this form. Business Hours:

We are closed on Saturdays, Sundays, and Japanese national holidays.

Japanese Holiday Periods:

• Year-end and New Year holidays

• Golden Week

• Obon

• Silver Week

I understand that doing so may result in immediate termination of the service without a refund, and I may be refused future services.

✓Changes after interpreter confirmation incur a 10%+ cancellation fee. However, changes to Medilator-arranged hospital appointments due to the hospital’s circumstances are free of charge.

✓Cancellations after a proposed hospital appointment incur a 10% booking service fee, deducted from the refund.

✓I understand that medical costs are not included in the interpretation fee.

✓I understand that Medilator provides interpreting services only to the person who made the reservation and does not handle complaints or negotiations with hospital or clinic staff. I agree to bear any related costs.

✓I understand that Medilator may cancel the service if the assigned interpreter becomes ill. Medilator will first seek a replacement or online interpreter; if unavailable, the full service fee will be refunded.
